If you are under the age of 18 and wish to start gainful employment (e.g. an apprenticeship), you must first undergo a medical examination (initial examination). To do this, you must first apply for an examination authorization certificate from the local authority. You will also receive a questionnaire. You must bring the documents with you to the doctor's examination and hand them over.
The initial examination is intended to prevent your health or development from being endangered by your employment.
After the examination, you will receive a medical certificate. You must present this to your employer before you start work. The certificate must not be older than 14 months at the start of employment.
Between the tenth and twelfth month after taking up employment, you must undergo a first follow-up examination and submit the certificate to your employer again. The purpose of the follow-up examination is to determine whether your employment has affected your health. The follow-up examination can then be repeated voluntarily every year until you are 18 years old (further follow-up examinations).
You do not need a check-up if you start a minor job or if you only work for a short time (maximum 2 months). This applies, for example, to a vacation job or a student internship.
The examination costs are paid by the state. You must apply for the examination authorization certificate and hand it over to your doctor so that he/she can reimburse the examination costs.

- This work is not marginal and lasts longer than 2 months.
Apply online:
Go to the online application at https://www.untersuchungsberechtigungsschein.de/, log in with your ID document with activated online ID function and enter the other data. After submitting the application, you will receive the examination authorization form (UBS) with the so-called UBS ID and the fillable questionnaire to prepare for your doctor's appointment within a few seconds. Both documents can be saved and printed out.
Apply on site (if not possible online):
Go to your local citizens' office or make an appointment for the issue of the examination authorization certificate. You must bring an identity document with you to the appointment. You will receive the examination authorization certificate with UBS ID and the fillable questionnaire to prepare for your medical appointment
Medical examination:
You may decide for yourself which doctor you wish to be examined by.
Make an appointment with your doctor. Show your UBS ID (cell phone or UBS printout) at the doctor's office. We also recommend that you print out the questionnaire for the initial examination, have it completed and signed by your legal guardian so that you can present it at the medical examination. This will speed up the examination.
You will receive a certificate after the examination. You should present this to your employer before you start work.
If you are still under 18 one year after the initial examination, you must be examined again. You will need another examination authorization certificate for this.
