The Bremen Health Department, in cooperation with the Bremen Public Order Office, is responsible for alternative practitioners in the city of Bremen.
Registrations for the examinations are accepted by the Bremen Public Order Office, in Bremerhaven the Citizens' and Public Order Office of the City of Bremerhaven is responsible.
The examinations for the general non-medical practitioner license for Bremen and Bremerhaven are carried out at the Bremen Health Department.
At present, written examinations take place twice a year:
- 3rd Wednesday in March
- 2nd Wednesday in October
This is followed by the oral-practical examinations approx. 4 to 8 weeks later.

The examinations consist of a written and an oral-practical part, for which only those applicants who have passed the written part are admitted.
The examination is not an examination in the sense of a performance check to determine a specific qualification, but serves to prevent risks to the health of individual citizens and the population. According to the Heilpraktikergesetz, the question of whether the license would pose a risk to public health must therefore be investigated. This means that, in addition to knowledge of the relevant health regulations, basic medical knowledge must also be examined, without which medical activities could easily be associated with risks to human health.

In addition to the general Heilpraktiker license, it is possible to apply for the following sectoral Heilkundeerlaubnises in Bremen:
- Permission to practice medicine professionally in the field of psychotherapy
- Permission to practice medicine professionally in the field of physiotherapy
- Permit for the professional practice of medicine in the field of podiatry
